The 2008-2009 Open Enrollment Forms and information are available below. Click on the title to view and print each item. Open Enrollment packets will be mailed out on Friday, May 9, 2008. If you do not receive an Open Enrollment packet you may download the forms and information contained in the packet below. If you have any questions regarding Open Enrollment please call the administration office at (866) 553-8206 or send an email to openenroll@admin-serv.com. You must enroll by June 13, 2008.

Before making a Plan selection, carefully read and review the health benefits Open Enrollment Guide to ensure that you understand everything fully and make a qualified Plan selection, suited to you and your family's needs. Use the various informational resources outlined in the Health Benefits Open Enrollment Guide if you have any questions or concerns.
2008-2009 Flexible Benefits Enrollment Form for Full-time Employees
Ensure that you fully complete, sign and date this form and make a copy of both sides for your records prior to sending it in to the Trust. If you are electing HCRA be sure that the amount you enter is the amount that you want deducted from your paycheck monthly. Reminder: This form must be faxed or postmarked by June 13, 2008.

Imputed Income Rates 2008-2009 Plan Year
Review the important Imputed Income information if you are covering a Same-sex Partner or Same-sex Partners' Child(ren). This will affect the amount of taxes that you will pay from your paycheck.
